About Bevacizumab
What is Bevacizumab? Bevacizumab is a recombinant humanized monoclonal IgG1 antibody that acts as a vascular endothelial growth factor (VEGF) inhibitor. Approved by the US FDA in 2004 under the brand name Avastin, it was the first clinically approved anti-angiogenic agent. By targeting the processes that allow tumors to develop their own blood supply, Bevacizumab changed oncology treatment, establishing angiogenesis inhibition as a standard therapeutic modality. Bevacizumab is administered via intravenous infusion, typically in combination with standard chemotherapy regimens. It is widely used in the treatment of several advanced and metastatic solid tumors, where it helps inhibit tumor growth, normalize tumor vasculature, and improve progression-free and overall survival. Mechanism of Action Vascular endothelial growth factor A (VEGF-A) is a key chemical signal produced by tumor cells to stimulate angiogenesis—the formation of new blood vessels from pre-existing vasculature. Tumors require angiogenesis to grow beyond a few millimeters in size and to facilitate metastasis. VEGF-A binds to its receptors (VEGFR-1 and VEGFR-2) on the surface of endothelial cells, activating a signaling cascade that promotes endothelial cell proliferation, migration, and survival. Bevacizumab binds selectively to the soluble isoforms of human VEGF-A, preventing VEGF-A from binding to its receptors on endothelial cells. By neutralizing VEGF-A, Bevacizumab inhibits the proliferation of endothelial cells and reduces the formation of new blood vessels. This deprives the tumor of the oxygen and nutrients required for growth. Additionally, Bevacizumab helps decrease the high interstitial fluid pressure within the tumor by normalizing the remaining tumor vasculature, which improves the delivery and efficacy of co-administered chemotherapeutic agents. Pharmacokinetics Because Bevacizumab is a monoclonal antibody administered intravenously, it has 100% systemic availability immediately following infusion. Its pharmacokinetics are characterized by a low clearance, a limited volume of distribution (approximately 3 to 4 liters, consistent with plasma volume), and a long elimination half-life. The terminal half-life of Bevacizumab is approximately 20 days (range 11 to 50 days), which allows for dosing intervals of 2 to 3 weeks. Bevacizumab is metabolized via receptor-mediated clearance and non-specific endocytosis in reticuloendothelial cells, rather than through hepatic metabolism or renal excretion, meaning dosage adjustments are not required for patients with renal or hepatic impairment. Clinical Indications & Dosage Bevacizumab is indicated, usually in combination with chemotherapy, for the treatment of: Metastatic colorectal cancer Non-squamous non-small cell lung cancer (NSCLC) Glioblastoma (treatment of recurrent disease) Metastatic renal cell carcinoma Advanced epithelial ovarian, fallopian tube, or primary peritoneal cancer Persistent, recurrent, or metastatic cervical cancer The standard dosage of Bevacizumab ranges from 5 mg/kg to 15 mg/kg administered intravenously once every 2 or 3 weeks. Dosing depends on the specific cancer type and the accompanying chemotherapy protocol. It must be administered as an IV infusion, starting with a 90-minute infusion, which can be shortened to 60 and then 30 minutes if well-tolerated. Warnings & Safety Warnings Bevacizumab is associated with serious, class-specific adverse events due to its systemic anti-VEGF effects: Gastrointestinal Perforation: A serious, potentially fatal complication occurring in up to 3% of patients. Bevacizumab must be permanently discontinued in patients who develop GI perforation. Wound Healing Complications: Anti-VEGF therapy impairs wound healing. Bevacizumab should not be initiated for at least 28 days following major surgery, and surgical wounds must be fully healed before starting therapy. It must be suspended before elective surgical procedures. Hemorrhage & Thromboembolism: Increased risk of severe, sometimes fatal hemorrhage (including hemoptysis in lung cancer patients) and arterial thromboembolic events (such as stroke or myocardial infarction). Hypertension & Proteinuria: Severe hypertension is common and must be controlled with antihypertensive therapy. Regular monitoring for proteinuria is required, and therapy must be suspended for nephrotic syndrome. Frequently Asked Questions Can Bevacizumab be used for eye conditions? Yes. Although not FDA-approved for ophthalmic use, Bevacizumab is widely used off-label via intravitreal injection to treat wet age-related macular degeneration (AMD), diabetic retinopathy, and macular edema. It inhibits abnormal blood vessel growth and leakage in the retina. What are the signs of GI perforation I should look out for? Patients should seek immediate medical attention if they experience severe abdominal pain, persistent nausea, vomiting, fever, or constipation during Bevacizumab therapy, as these can be symptoms of gastrointestinal perforation.
